I have attached several forms for you to fill out and return. (wholesale pricing & credit application)

If you are interested in applying for credit, we will require three active trade references and one bank reference. Otherwise, we can take a pre-paid order with your Visa or MasterCard. Lisa will need the account number, the following 3-4 digits (usually located on the back of the card), the expiration date, and the billing name and address associated with the card, together with your order.

All future orders will need to be placed in writing either via our fax 360-576-0065 or at her email, orders@bi-o-kleen.com.

Upon completion of setting you up as a new customer, please contact me, and I will forward you some data about Bi-O-Kleen that will be acceptable for you to use on your website, including a user agreement that we ask our internet and mail order customers to sign that protects Bi-O-Kleen logos and other copyright information. You will be able to use picture shots on your website and we also have some text you can put up as well that gives a product description and use.
